So what do Occupational Therapist do?

Occupational Therapists help people to live life their way. When physical or mental illness, or the effects of ageing stop you doing some activities – whether it’s getting washed or going back to work, or doing the fun things such as socialising – this affects your health and wellbeing. We can work with you to find different ways of carrying out activities to help you live life your way and we will be bringing some of our small equipment to your surgery in the morning to show you how. Drop in and say hello and find out about our service.

Online Registration With The Practice

Please note that if more than one family member is to be registered, you will need to fill out this form(s) for each person.

When you register you it is important to fill in this medical questionnaire. This is because it can take a considerable time for us to receive your medical records. 

In person Registration With The Practice

You will need to supply us with proof of ID (any document with your name and address on), we can then take a copy of the original for you when you are attending in person.
